- An institution to be designated as a registry agency shall complete the Appendix Form 2 Application for Designation of Registry Agencies for Advance Directives and directly apply for registry agency designation through the system operated by the managing agency.
Documents to be submitted when applying for registry agency designation
Documents to prove that it is a target institution
- Regional Healthcare Institutions under Article 2 of the ˹Regional Public Health Act˼
- Documents proving that the institution is a health center, medical center, branch office of community health center, or life support center as registered in the regional healthcare system
- Medical Institutions
- Establishment permit for the medical institution

Non-profit Corporations or Non-profit Organizations that provide services concerning advance directives on life-sustaining treatment

- Choose one in certified copy of register of corporation or business registration certificate
Documents to confirm designation requirements
- Documents that can confirm the location and size of the office and counseling room(certified copy of register, etc.)
- Four or more photos taken within one week of submission date so that the size and relationship of each office and counseling room, security measures, and boundary with surrounding space can be confirmed
- A list containing the types and quantities of processing devices(or processing environments), such as PCs for work processing, scanners, internet, etc., and basis photos taken within 1 week of submission date

- The Minister of Health and Welfare may conduct an on-site check at the site if necessary to review the application for designation of a registry agency, and may request submission of documents or opinions to the relevant central administrative agency or local government head.

- At this time, the Minister of Health and Welfare may have the head of managing agency check the documents and local sites for the review of application for designation and report the results.
- The Minister of Health and Welfare shall review the submitted documents and the site confirmation results to designate a registry agency and decide whether or not to designate comprehensively considering distribution by region, budget situation, policy direction, etc..
